Our team has dedicated Palliative Care Nurse Specialists who can visit children and young adults at home and in the community. The team will cover our catchment area, from Merseyside to the Wirral and Cheshire to North Wales.

What can families expect from support in the community?

When a Palliative Care Nurse Specialist from Claire House visits, they will spend dedicated time getting to know the child or young adult and the family involved.

While building a relationship, they will also understand the family’s wishes for the child in the now, while also planning for the worse case scenario. The role of our Nurse Specialists is to be there to support the family, providing medical and emotional support where needed.

Our team helps families understand the options available in managing a child’s current condition, as well as being there for any changes in the future and incorporate this into their advanced care plan. Along with this, the team can attend hospital appointments where needed to help provide medical guidance and emotional support.

By working closely with the family and other medical professionals, our team can also make referrals to other services at Claire House.
