Hospital Support

Our team of Palliative Care Nurse Specialists work closely with the hospitals in our catchment area to be there for the families of seriously and terminally ill children.

Our team takes the time to visit families referred to Claire House within the hospital, building a connection and understanding of the child’s needs and what matters most to the family. 

For families who are new to Claire House through the hospital referral, our Palliative Care Nurse Specialists dedicate quality time to help establish a strong, trusting relationship. We’re here to support families in those difficult moments—whether it’s guiding them through challenging conversations or assisting with discharge planning.

We understand that hospital stays, especially in critical care units, can be unpredictable, and we want to help families feel more at ease by offering support, no matter what the future holds.

By providing parallel planning, we aim to balance hope with ensuring families feel supported throughout their journey.

Claire House will be there with you every step of the way. When your child is ready to leave the hospital, we’re here to offer options and support for the care your family needs, both in the short and long term.

Key Benefits of Hospital Support:

Through our close relationship with Alder Hey Children’s Hospital, we now have a dedicated team member who spends time each week on their critical care unit and will be made aware of children or young adults who may need support of Claire House.

End-of-life Support

Sadly, if the time comes for end-of-life care, the relationships we’ve built with families allow us to offer compassionate guidance and support. 

Our Palliative Care Nurse Specialists will help families understand their options, so they can make informed decisions about where their child will be cared for—whether at home, in the hospice, or in the hospital. No matter the choice, Claire House will continue to provide care and support throughout the end-of-life process and into the grief journey that follows. 

Our care team is equipped with the specialized skills to manage complex symptoms, ensuring that children are as free from pain and distress as possible. We also offer emotional and practical support, walking alongside each family as they navigate these challenging moments.
